How to Apply for Meals on Wheels

Meals on Wheels Information

Call Michelle McMahon at 941-6416, she is the Coordinator for Colden Meals on wheels. She can be reached between 9:00 a.m. 6:00 p.m., Monday through Friday. Be prepared to give the following information:

Full Name
Complete address
Phone Number
Date of Birth
Marital Status
Two contact persons who can be contacted in case of emergency, including their address and phone number
Physician's name, complete address and phone number
Client's Diet

COST OF MEALS:

There is a suggested, voluntary, confidential contribution of $6.25 per day for two meals, or $30.00 per week. No one will be denied service because of an inability to contribute the full amount. However, Meals on Wheels depends on client contribution for one-third of their budget.
