ADHT offers state-of-the-art integrated telehealth services by providing  in-home patient monitoring of vital signs (blood pressure, blood oxygen, pulse, blood sugar, weight, and medication compliance) for patients with chronic diseases such as Diabetes, Hypertension, CHF (Congestive Heart Failure), COPD (Chronic Obstructive Pulmonary Disease), and Obesity. Vital signs data is obtained using state-of-the-art, easy to use, wireless/Bluetooth enabled devices and a pill station; and is monitored in our downtown Buffalo monitoring center by LPN’s who interact with patients requiring follow-up phone calls to discuss their health status. Triage staff provides coaching, patient support and interventions as appropriate.

The American Red Cross, Serving Erie & Niagara County's Services to Seniors Department offers several programs to assist area seniors and their caregivers, including the Ombudsman program, when volunteers visit residents of long-term care facilities and act as their advocate. We also provide support to caregivers by offering the Chronic Disease Self-Management program, known locally as the Living Healthy program. This is a six-week long evidenced based intervention that helps support older adults living with chronic illness. The CARES Program is a service available to the patients of the UB School of Dental Medicine that can help patients to identify concerns which might affect their ability to receive dental care. Through referral, CARES staff assist patients with obtaining insurance, housing, transportation, food stamps, health care, and mental health care. The CARES Program also provides its patients with information related to education, job training, caregiving resources, stress management, financial assistance, health care proxies or living wills, and other services our patients need. The CARES Program can also serve as an advocate for their patients as they complete dental care at the UB Dental Clinics, by assisting in communication with dental students and faculty.

Provides a social model program for the physically frail, functionally impaired, socially isolated and cognitively challenged.

A limited number of subsidized participation fee slots are available for qualifying seniors.

The Total Aging In Place Program is a managed long term care plan (insurance plan) and a provider of long term care services. The program is designed to help its members to continue to live safely in the community. The program provides and coordinates needed health care services that participants can receive at the program's Day Center or in their home/apartment such as: home health care, physical/occupational therapy, social work, nutrition, non-emergent medical transportation, home delivered meals, durable medical equipment, Optometry, Podiatry, Audiology, Dentistry, personal emergency response systems, and social day care. Total Aging can be paid for through Erie County Medicaid or Private Pay.
