Alzheimer's Proofing Your Home Program
(APYHP)

Request a free, confidential in-home assessment by a trained professional that will:

  • Provide a personal walk through of the home to note areas of concern
  • Demonstrate assistive devices available to make caregiving easier
  • Assist caregivers of the memory impaired or those with dementia
  • Enhance the safety of elderly in the home
  • Help the elderly and caregivers link to broader service system
  • Provide a customized report with suggested home modifications
